Abstract
Within home-based care, the palliative care of patients with advanced and incurable diseases occupies a very special place. The clinical case herein presented addresses the palliative care of a 76-year old patient with advanced cancer disease, tracheotomy and gastrostomy in the home setting. After performing a comprehensive assessment, different nursing diagnoses and collaboration problems were identified and a care plan was implemented, prioritising care of the ineffective cleaning of the air pathways and pain management. Over the course of the care process, communication, emotional support, ongoing assessment and family involvement, in this case the patient’s wife as the primary carer, proved to be essential.
