Abstract
The admission/transfer of patients with chronic diseases (convalescent patients, palliative care, rehabilitation, etc.) to the healthcare module most suitable to their needs, has been traditionally difficult given the ineffective communication amongst the different healthcare centres. We, here at Hospital Clínico, aware of the reality have created a "key" figure: "the Referral Manager", whose mission is to develop a specific programme destined to relocate patients (already admitted in our centre) to other support hospitals equipped to care for the specific health problems of these patients. After the first year of operation, we have been able to substantially improve the referral process of these patients, by involving our professionals and thus obtaining professional satisfaction, improving the quality of the service rendered to our clients and by becoming more effective in the delivery of care from both a clinical and organisational perspective.
