Abstract

Introduction: an effective management of resources requires the identification of the nursing product and its cost. The objective is to determine the cost of the nursing product in domiciliary primary care by identifying the user’s profile and the most common nursing diagnoses and nursing interventions.
Material and method: descriptive observational study conducted in the Area 12 of Madrid. 649 users of the Domiciliary Care programmed were included from 1/05/2000 to 31/05/2001.
Results: a total of 4885 home visits were performed (mean 7.5 visits per user and year).
There were 5,317 nursing diagnoses (mean 8,2 diagnoses/user) and 42,993 interventions (mean 8,8 interventions per visit and user). Mean time for home visit, taking into account travel time, the time spent at the residence, and the preparation of the visit, was 55,1 minutes. The cost of the nursing home visit was: total cost per patient (including travel time and cost, preparation and performance of the visit) 92 euros, total cost per visit, total cost per visit: 12,22 euros, total cost per intervention: 1,39 euros.
Conclusions: the utilisation of a common language (PFSMG-NANDA-NIC) in the process of nursing care facilitates the assessment, diagnosis and treatment of disease and health conditions looked after by nurses. NIC taxonomy is very beneficial to group treatments and to homogenise care. The overall costs of Domiciliary Primary Care are very small in relation to the total cost of the care received by primary care users.
