    Abstract

    Introduction. This study aimed to assess the prevalence and prognostic impact of frailty in elderly patients in the acute cardiac care unit.

    Material and method. Patients over 65 years old and consecutively admitted to the acute cardiac care unit for three months were included. Demographic and analytical data, geriatric and cardiological tests and scales, in-hospital evolution, and complications were collected.

    Results. 105 patients were included, mean age: 77.8 years, 66% men. The most frequent admission diagnoses were acute coronary syndrome (29.5%), atrioventricular block (24.8%), acute heart failure (15.25%), and post-structural heart intervention (14.3%). The prevalence of frail patients was 22.9%. Frailty was associated with general aspects such as older age, female sex, worse functional status, lower hemoglobin levels, and glomerular filtration rates at admission, lower triglyceride levels, longer hospital stay, and worse quality of life. Frailty was also associated with geriatric assessment i.e., dependency, cognitive deterioration, depression, and polypharmacy, and with nutrition i.e., worse nutritional status, and lower body mass index, albumin, calcium, and circulating iron levels. Lastly, frailty was associated with in-hospital complications such as heart failure, phlebitis, acute kidney injury, and increased risk of in-hospital mortality.
