Contact email: alfra_serolf@hotmail.com Abstract Heart failure is a syndrome that afflicts elderly people who experience a significant deterioration in physical functioning and must manage medical regimens that involve the modification and adoption of healthy life behaviors and this can be made difficult by various sociodemographic factors that limit people to following care recommendations at home.We present the case of a patient in the fourth decade of life with a medical history of antiphospholipid syndrome, with a post-operative mitral valve replacement. After days of assistance with extracorporeal membrane oxygenation and stabilization, the patient leaves the critical care unit to continue her recovery, rehabilitation, and training in adopting new health behaviors necessary to maintain her physical condition at home. The assessment of the person is carried out through the 14 needs of the Virginia Henderson model, identifying those most altered for training and learning that give rise to the development of nursing diagnoses; For planning, the person‘s strength limitations are considered, and the execution of the interventions is established under these premises, obtaining a satisfactory evaluation. For the results, the taxonomies of the Classification of Nursing Outcomes and the Classification of Nursing Interventions are used.This article aims to present the execution of a care plan with comprehensive follow-up interventions for a young patient with acute heart failure.
