Abstract

The AREA model has been applied in the care process presented in this article. The care process has been conducted on a breast-feeding baby girl (María) included in the Healthy Child programme, organised by a nursing consultation office in Primary Care. The information obtained results from the interaction amongst patient-family-nurse. The initial assessment of the baby detected the need to work with the mother (Carmen) as she was the direct carer providing basic care to the baby and was also the informal care giver of her own mother (Amparo, the baby´s grandmother) who suffered from dementia.
Assessment: the M Gordon assessment guide was used in both cases. Golberg´s scale was also used to monitor anxiety and the Coop/Wonca sheets to ascertain the functional status of the main carer.
Diagnosis: as a result of the clinical reasoning Carmen was diagnosed with two main conditions: carer fatigue and a disposition to improve the parental role. María was given the well-being diagnosis of
“Effective breastfeeding”. Expected outcome: the following outcome was envisioned for Carmen: well-being of the main carer giver and functioning of the family. As for María, results that allow her child development to be assessed over time are established.
Interventions: The main interventions aimed at the attainment of the expected results for Carmen were: Support to the main care giver and paternal education: bringing up of children by nuclear family. In María´s case, standard interventions from the nurse´s consultation office were applied.
Assessment: Assessment is performed through the indicators derived from the main proposed results in both cases and considered to be appropriate, enabling a comparison between the responses obtained and the outcome criteria intended to assess progress.
