Abstract
Chronic diseases are long lasting and usually with slow progression. This work focuses on four high-prevalence chronic diseases: diabetes mellitus, hypertension, dyslipidemia and metabolic syndrome, of which we will present their epidemiological, prevalence, incidence and mortality data as well as mechanisms of prevention and control that the nurse should take into account in their clinical practice.
The control and prevention of cardiovascular diseases and risk factors associated with them have been stated as a priority area, hence, health professionals, mainly from primary care, have a key role. To achieve positive changes in the management of chronic diseases it is necessary to combine all activities undertaken by the professional nurse, for its prevention, promotion and control, as well as the activities of the patients themselves.
