Abstract

We present clinical case between the hospital and primary care centre of an elderly patient with Alzheimer\'s disease whose primary caregiver, his wife, is also elderly. The hospital nurses attempt to coordinate the case with the primary care nurses to develop intervention strategies that can help solve the problem, supported by the services offered by the Law on Promotion of Personal Autonomy and Care for Dependant People. A social and health approach is needed to solve this case, and the nurse plays the role as a link between the two institutions.
