Abstract

Introduction: the midwife is an essential professional in the area of sexual and reproductive health throughout the life of women, as well as for the integral health of newborns until 28 days after birth. The midwife is the healthcare professional better qualified, entitled and cost-effective for the care of women and their newborns. It is a reality that there is great variability in care during pregnancy, delivery and postpartum, with high prevalence of medicalization during this stage, and limited models for continuity of care led by midwives, which we believe is the more adequate model of care for boosting maternal and fetal wellbeing during pregnancy, delivery and postpartum. Objective: maternity care is the competence of midwives; therefore, the aim is to understand the impact of midwife during pregnancy, delivery and postpartum on maternal-fetal and neonatal health outcomes, breastfeeding, perinatal mental health, and economic costs, compared with other models. Method: an umbrella review was conducted; there was a search for articles in the following databases: Cochrane Library, Web of Science (WOS), Scopus and Cuiden, using the key MeSH terms “midwife”, “continuity of patient care”, “pregnancy”, “newborn”, “perinatal mental health”, “breastfeeding”, “health care costs”. In order to be selected, these articles had to be bibliographic, narrative, systematic or exploratory reviews, or meta-analyses, without limitations in terms of time of publication, until November 2023. The PRISMA methodology was applied. Results: nineteen (19) reviews were evaluated, including n=228,509 pregnant women in total, n= 72,388 women during postpartum, and n= 7,931 midwives. These reviews were from Australia, Iran, Spain, United States, United Kingdom, Mexico, Canada and China. Care by qualified midwives during pregnancy, delivery and postpartum increased the rates of normal deliveries and reduced the need for interventions, there was an improvement in the results for initiation and continuation of exclusive breastfeeding until 6 months of age, as well as an improvement in maternal perinatal mental health, and savings in healthcare costs were demonstrated. Conclusions: the model of care for maternity led by qualified midwives is safe, improves the health of women and their children, and is cost-effective. These reviews were analyses about midwife care for low-risk women, and further research is required in order to assess the impact of this type of care in women with medium and low risk.
