Abstract

Introduction: Cardiac catheterization by radial access reduces vascular complications and improves patient´s condition. Whether performing it either by the right or the left radial artery has been evaluated in several studies, but not when it is related to hemostasis and its complications. The aim of the study is to determine whether hemostasis and its complications will vary depending on how it is performed, whether through the right or the left radial artery, and, secondarily, whether or not it is more advantageous to perform it through the left one.
Method: Randomized prospective observational study. 200 consecutive patients were randomized for left or right radial catheterization. All patients with a positive Allen test were included. For hemostasis, the pneumatic bracelet was used, and the unit’s protocol was used for its decompression and subsequent removal. Twenty four hours later, the puncture site was evaluated and the inverted Barbeau test was performed so as to confirm arterial permeability. Variables were analyzed and compared with the use of the Chi square and Student´s t tests.
Results: Overall complications accounted for 10%. Through the right radial artery in 6% and through the left radial artery in 14%, there was no statistical significance (p=0.31). There was an occlusion in the right and none in the left radial artery, and the rest of the complications were minor. As for the performance of the procedure, there were no differences between both groups.
Conclusions: There were no significant differences when performing hemostasis through the right or the left radial artery, it being effective in both. Likewise, in our setting, performing catheterization by the left access did not show any advantages.
