Abstract

ntroduction. Pulmonary thromboembolism (PTE) poses an important health issue in our environment. Various studies evidence gender differences concerning PTE, the majority of them being performed in urban areas.
Objective: To describe the sociodemographic and health care differences existing between women and men suffering from PTE in a rural hospital of Area I of the Principality of Asturias.
Method. Cross-sectional study carried out at the Hospital of Jarrio. Variables relating to sociodemography, clinical presentation, use of the health system and health care received were analyzed through the computerized medical records of those patients who suffered from a PTE in the years 2013, 2014 and 2015.
Results. 116 PTE incidents were studied. The majority (59.5%) of them occurred in women, who had a higher mean age when suffering from PTE (p<0.001). Men were heavier smokers and alcohol consumers (p<0.001). Women suffered more from arterial hypertension (p=0.003) and obesity (p=0.008). Men had a higher prevalence of acute myocardial infarction (p=0.006) and COPD (p<0.001) as clinical history. With respect to health care, we can observe that women are moved less often to another, higher-level, hospital center and undergo less diagnostic tests, such as echocardiography (p=0.024). During admission, women present more complications, especially neurological ones (p = 0.038), and a higher mortality.
Conclusions. There are gender differences in demographic variables, in cardiovascular risk profile, in clinical presentation, in hospital management and in the co-morbidity/mortality of PTE in area I population, which suggest areas for improvement.
