Abstract

      Patients with adverse clinical characteristics, poor ventricular function, complex coronary disease who are turned down for surgery usually undergo percutaneous coronary intervention (PCI). These complex PCI procedures require the use of temporary percutaneous left ventricular assist devices. Impella® is currently the most common left ventricular support device for high-risk PCI.
    The following work describes the nursing care plan for a 68-year-old patient undergoing high-risk catheterization using Impella® ventricular support and rotational atherectomy.
For the evaluation of the care plan, nursing diagnoses and potential complications were described according to the NANDA-NIC-NOC taxonomy. The most significant results (NOC) and interventions (NIC) were identified; also, all the activities were described in detail. It was observed that the following complications proved to be the most relevant: risk of bleeding, decreased cardiac output, risk of peripheral neurovascular dysfunction, risk of infection and anxiety.
The nursing team is responsible for the patient’s first contact in the catheterization lab. Therefore, it is essential that nurses know how to identify the needs of the patient and their relatives. Moreover, appropriate training and preparation is also necessary to perform complex procedures and techniques.
