Systematization and implementation of a diabetes education program at the initial onset of diabetes in a pediatric inpatient unit

Abstract

Introduction: all children with newly diagnosed type 1 diabetes mellitus should be admitted to the hospital for treatment initiation. The incidence of this disease in Spain is approximately 15-20 per 100,000 children under 15 years of age. Given that the population of Fuenlabrada is 209,000 inhabitants, the incidence of diabetes onset in the pediatric hospital is expected to be high. Given that in the Hospital of Fuenlabrada there is no nurse in the pediatrics unit who could train in diabetes, such education is provided by nurses and endocrine staff from the pediatric hospitalization ward. This is the reason it seemed necessary to train the nursing staff on inpatient diabetes education and their level of survival, through theoretical and practical courses. At the same time, a computerized clinical record was developed, which will guide the nurses and pediatric endocrines when providing diabetes education to pediatric patients who present type 1 diabetes.
Objectives: to provide education in diabetes in order to achieve a higher quality of life in the shortest possible time to the children admitted due to onset of diabetes; to know how nurses value this type of courses and their impact on their level of knowledge.
Methods and materials: design of a computerized medical record on diabetes education that was developed following the guidelines of diabetes education for survival. Training Program consisting of:

  • A course for the registered nurses which was conducted in two identical sessions in order to maximize the number of participants from the pediatric area. The date on which it was conducted was october 2007 with a total duration of 10 h per session.
  • A course for nursing assistants, in june 2008 for a total duration of 8 h.
  • The course presented the various stages of registration for a better understanding.
  • ECODI questionnaire about diabetes knowledge, completed at the beginning and at the end of the course to measure the knowledge acquired.
  • Satisfaction survey of the courses. The course and teaching staff evaluation was performed by an anonymous survey, with a scale of 1 to 10.

Results: the training was given solely by the pediatric hospital nurses to a group of 14 registered nurses and 10 nursing assistants. The increase in the level of knowledge after completing the course was statistically significant (P <0,05).
The students gave the course and the teachers a score of 8,5.

Conclusions: with the training of all the staff of the unit in the management of patients with onset of diabetes and the use of the form as a methodological tool, there was an increase in the knowledge and skills of the staff in the subject of diabetes education. A computerized medical record on diabetes education was implemented in october 2007.
