Status of the protocols of normal delivery care in public hospitals in the Valencian Community

Abstract

Normal delivery is defined as the unique physiological process in which the woman ends her pregnancy to term, in which are involved psychological and sociocultural factors. Its onset is spontaneous, and it evolves and finishes without complications, culminating in the birth and does not need any intervention other than full support and respect of the process (Federation of Spanish Associations of Midwives FAME, june 2006).
In recent decades there has been a widespread medicalization of pregnancy and childbirth. There has been a rapid expansion in the development and use of multiple practices designed to initiate, increase, accelerate, regulate or monitor the physiological process of childbirth, in order to improve outcomes for mothers and their children. In developed countries, where such practices have become common, some people are beginning to question the value of these high levels of interventionism.
For these reasons, we performed a literature review in order to choose the most comprehensive document for the formulation of a protocol for normal delivery, and then analyze the current situation of teaching public hospitals in the Valencian Community.
An analytical, observational and transverse study was carried out comparing the existing protocols in the fifteen hospitals in the Valencian Community regarding the strategy of care in normal deliveries in the National Health System.
The study included data from November 2007 to april 2009.
The collection and analysis of protocols were carried out between june 2008 and april 2009. It was a preliminary study before the implementation of a delivery protocol for the Department of Health of the Valencian Community in 2009, following guidelines from the Ministry of Health.
Of the fifteen hospitals studied, nine had protocols/procedures for delivery.
There were no hospitals that met 100% of the recommendations of the Strategy of Care of Normal Delivery in the National Health System.
There were two hospitals with high percentages (83,33% and 45,83%), and six hospitals with 0% (for the absence or inaccessibility of protocol). Seven hospitals were in the range from 4,17% to 29,17%. In conclusion, we observed a lack of normal delivery protocols in the Valencian Community during the study period.
