Abstract
Relationships between head circumference, birth weight, and type of delivery in nulliparous women
Introduction: current scientific data show head circumference and birth weight to be good markers for cephalopelvic discrepancy and difficult delivery. Moreover, excessive weight gain and a high BMI (body mass index) are associated to complications in pregnancy, birth, and postpartum; and result in a high birth weight.
Purpose, material, and methods: the present investigation aimed at establishing a potential relationship between neonatal endpoints (head circumference and birth weight) and type of delivery in primiparous women (580) at Complejo Hospitalario Universitario de Albacete (CHUA).
An observational analytic case-control study was carried out in a sample of 580 women and their sons/daughters.
Results: the association of birth weight with type of delivery has a chi-squared value of 0.011, with a statistically significant difference. OR for a birth weight higher than 3.500 is 2.37 for C-section risk and 1.28 for instrumental delivery.
Regarding association of head circumference with type of delivery, a head circumference higher than 36 cm was associated with a 2.23 risk for C-section and a 1.22 risk for instrumental delivery. Chi-squared value was 0.006 for both endpoints.
Conclusions: a statistically significant association was found for head circumference (HC) > 36 cm and for birth weight > 3.500 g with the type of dystocial delivery (instrumental delivery or C-section). Conversely, HC < 34 cm and weight < 2.500 are protective factors and are associated to more likely normal delivery.
