Abstract

Home hospitalization is a resource which is being used more and more frequently, requiring the extraordinary and specific coordination of different institutions and professionals. This article presents the process of a specific home hospitalization case that was carried out over the course of one month; starting when Kevin, a one-year old patient, was moved from the Intensive Care Unit (ICU) of the Puerta del Mar Hospital (Cádiz), to his home, located in a rural area with limited resources.
It was the first time that, as a case manager nurse. I was faced with such a complex and novel case. The management of the team required and the coordination between the different agents involved, as well as an accurate assessment of the home and the patient’s parents’ training were extremely important procedures to reach the main objective: to move Kevin to his home.  This objective was fulfilled thanks to a process that was coordinated and led by just one person: the case manager nurse. This case demonstrates that home hospitalization can be successfully carried out despite its complexity, improving the quality of life of the patient and his/her family.
