Abstract
Objective: to identify the criteria for referral of patients from the Emergency Nurse (EN) to the Community Nurse Case Manager (CNCM).
Material and methods: descriptive study using Delphi´s expert consensus technique which was conducted in primary care districts in the province of Huelva. The expert panel was composed of 22 CNCM and Nursing Coordinators (NC). The first questionnaire contained 139 items, distributed in 16 fields, corresponding to the 14 requirements of the Virginia Henderson´s model, a field for the scales and rates of assessment and one for the caregiver. Each field included an open field, which was analyzed with thematic categorization. The median of medians was used as cutoff point to pass from one questionnaire to another.
Results: of the 139 items of the initial questionnaire nine reached results. The median of medians was six for the first questionnaire, eight for the second questionnaire and 8,5 for the third questionnaire.
Conclusions: the results of this study provide nine nuclear criteria for referral of patients from EN to the CNCM. These nine indicators are:
oxygen therapy; feeding/parenteral nutrition; loss of continuity of the skin; compliance with the Pharmacological Therapy Plan, identity of the primary caregiver, Barthel Index, Pfeiffer Index, Caregiver Stress Index; readmitter.
