Abstract

Objective: adverse Drug Reactions (ADRs) represent a threat for the safety of hospitalized patients. The objective is to find out the incidence of Adverse Drug Reactions (ADRs) in a mid-
to-long-term stay hospital, to identify patient and ADR characteristics, and to determine ADR record in the Digital Clinical History prepared by professionals.
Method: a retrospective and descriptive study has been conducted in the Internal Medicine Department of the Hospital Gorliz (Biscay) from December, 1st, 2009 to December, 31st, 2011. With the relevant authorization by the Ethical Committee for Clinical Research from the Hospital Universitario Cruces, the competent authority, and the Hospital Gorliz management, there was access to the databases set up by the computer applications of digital clinical records: Global Clinic, Zaineri and eOsabide.
Results: the incidence of ADRs in patients hospitalized in this period was 13% (n= 238). ADRs were more frequent in male patients (57%), 40% of these appeared in 81-to-90-year old patients, and 95% were Type A reactions, mainly already known and described in the product specifications. The mean stay of patients with ADRs was significantly higher (P< 0.0001). The group of drugs for the digestive tract and metabolism was the cause for 31% of ADR cases.
227 ADR cases had been recorded in nursing reports, and 94 cases in clinical reports at hospital discharge.
Conclusions: polymedication, advanced age, and a prolonged hospitalization are associated with Adverse Drug Reactions.
