Abstract

Introduction: the transition process for patients from an Intensive Care Unit (ICU) is essential for providing safe and timely care. The im-plementation of this process has demonstrated efficacy for patient recovery, a reduction in hospital stay, and a reduction in readmissions and in the incidence of medication errors. These interventions involve transfer, supervision and focused programs, with a multidisciplinary and comprehensive approach throughout the ICU hospitalization process and until rehabilitation at short and long term for patients and their families.
Objective: to incorporate the evidence available on the interventions associated with the transition process managed by nurses, asses-sing their impact on readmission, hospital stay, and medication errors, as well as their potential impact on physical, psychological and cognitive consequences for patients and their families, subsequent to their transfer from an ICU.
Method: the review was conducted in the following databases: EMBASE, Cochrane Central Register of Controlled Trial, PubMed, EBS-CO-CINHAL-PsycINFO, Web of Science and Scopus, and a three-step methodology was followed, with the following MeSH terms: “Tran-sitional-care”, “patient-discharge”, “patient-transfer”, “patient-care- planning”, “critical-care”, “nurses”. After the retrieval, the complete text of those articles selected was read independently by two reviewers.
Results: findings were grouped into a narrative summary of the literature available. The results included four interventions to optimize the process of transition from the ICU.
