Clinical case: nursing care in a young patient with dilated cardiomyopathy

Abstract

      Dilated cardiomyopathy is a disease of the heart muscle which involves ventricular dilatation and decreased systolic function, leading to heart failure. It usually affects the left ventricle, although it can occasionally affect both ventricles. Dilated cardiomyopathy is usually diagnosed in middle–aged individuals, being rare in childhood. Dilated cardiomyopathy is the most common cause of heart failure and heart transplant.
We describe the clinical case of a 38-year-old man who presented with ventricular tachycardia as a consequence of undiagnosed dilated cardiomyopathy. After stabilizing the patient and performing catheterization and magnetic resonance imaging testing, an implantable cardioverter defibrillator was implanted to prevent sudden death. A care plan was developed following the conceptual model of Virginia Henderson and relying on the taxonomy of the North American Nursing Diagnosis Association, the classification of nursing interventions and the classification of nursing results, to achieve individualized, comprehensive and quality care. Several diagnoses were identified: decreased cardiac output, risk of constipation, risk of deterioration of the skin integrity, ineffective cleaning of the airways, anxiety, and risk of infection. The evolution of the patient was favourable, achieving most of predefined objectives.
The nursing team works to monitor possible complications and keep the patient informed at all times so that they can identify possible risks and reduce anxiety.
