Abstract
The Case Management Society of America defines case management as a collaborative process of assessment, planning, application, coordination, follow-up and evaluation of the options and services required to meet an individual’s health needs through communication and available resources to promote quality cost-effective results. This care practice is applied in Andalusia, under the name liaison nursing, to address the so-called socio-sanitary syndrome, a consequence of the socio-demographic and morbidity changes that have taken place in the last few years, and the increase in the care needs of the population.
The objective of this work is to present a review of the analogy between the Case Management model applied to care and the Specific Case Management nursing intervention pertaining to the Nursing Interventions Classification (NIC). This review aims to demonstrate how the specific logging and registry of the work carried out by the case manager nurse in care plans could facilitate the subsequent recovery of data in the information systems.
The fundamental principles of the Case Management practice model are presented and the similarities between the activities included in the Specific Case Management intervention and the case manager nurse’s functions and the liaison nurse’s activities, extracted from the Case Manager Nurse Manual in Andalusia, are identified.
