Abstract

Anemia is a common problem in patients admitted to the Intensive Care Unit. It has a multifactorial etiology, with increased transfusion requirements and higher morbimortality.
Objective: to ascertain the prevalence of anemia and the transfusion of post-surgical patients during the first five days of admission to the recovery room, and if there is blood loss related to the collection of blood samples for laboratory work.
Material and methods: a descriptive, observational and retrospective study of patients admitted to the postoperative recovery unit, from january to december 2008, at the “The Princess” University Hospital (Madrid). Post-surgical patients admitted to the recovery room for a period of no less than five days were included in the study. Those patients who had died during the study period, those who had undergone cardiac surgery, those with multiple trauma injuries and those with a tendency higher than usual to bleed were excluded from analysis. Data relative to anemia, transfusions and loss of blood volume and red cell mass from the withdrawals of blood samples for analysis were analyzed.
Results: the medical records of 65 in-patients who met the inclusion criteria were reviewed. The prevalence of anemia on admission is 86%, a 89.83% of the patients were transfused during the first five days of hospitalization, requiring an average of 1.20 erythrocyte concentrates per patient. There is a decrease in Hb of 1.32 g/dl (95% CI: 0.81 to 1.83 g/dl), despite being polytransfused patients, but there is no significant relationship between this and the blood drawn for laboratory purposes.  
Conclusions: anemia has a high prevalence on admission to the recovery room and during the first five days blood hemoglobin levels continue to decline, despite being patients with high transfusion rate. Blood loss due to blood drawing are constant during the stay, but no relationship has been found with decreased blood hemoglobin.
