Abstract

Purpose: to assess factors related to urinary tract infections associated with the use of a urinary catheter in patients in an Intensive Care Unit in Yucatan, Mexico.
Methods: an epidemiological, cross-sectional, case-control study of a cohort of 113 adult patients in the Intensive Care Unit of a tertiary hospital from June 2017 to June 2018. Data were collected from patients' clinical records and from hospital epidemiological surveillance reports. Data analysis was carried out in STATA 14.1. A 95% confidence level and a statistical significance level for a p value of ≤ .05 were used.
Results: 65.22% of women with a urinary catheter developed a urinary tract infection. The mean age was 46 years (SD 16.58). The use of chlorhexidine-impregnated wipes was a protective factor for urinary tract infections associated with urinary catheters (RM 0.07, 95% CI 0.01 - 0.47). The main risk factors for the development of this infection were APACHE II score greater than 25 points (RM 7.08, 95% CI 1.42 - 42.95) and use of a urinary catheter for more than 15 days (RM 6.71, 95% CI 1.48 - 30.37).
Conclusions: the use of chlorhexidine-impregnated wipes has a protective effect on the development of urinary tract infections. The main risk factors associated with the UTI-UC were the duration of the hospital stay, the severity of the medical condition at the time of admission (APACHE II) and the prolonged use of a urinary catheter.
